Representative Authorization Form

This form is required only if the candidate intends to send a personal representative to the All Candidates Meeting.

The All Candidates Meeting is mandatory. Failure to attend or send a representative (authorized via this form) will result in immediate disqualification for office.

In order to send a representative, this form must be submitted in full to the Board of Elections below twenty-four (24) hours prior to the start of the All Candidates Meeting.

Do not submit this form until you have completed the Intent to Run Form for the appropriate elections cycle.
